Student Medication

The school Nurse may store and administer a prescribed medication during the school day. Here are a few steps you may take to begin the medication administration process:

1. Bring the prescribed medication to the front office and request the Nurse
   1. The medication must be in its original container with the pharmacy or brand label affixed.
   2. The label must include: Student’s name, name of medication, dosage of medication, time of dosing and a physician/ prescriber name
2. A consent form must be obtained from the Guardian before any prescribed medication may be administered to the student.

3. All prescribed medication will be administered strictly in accordance with the written order of the prescriber. The dosage may be changed only if the school is provided with the written order of the physician authorizing the change. We cannot accept physician orders over the phone.

IC 20-34-3-18: Sec. 18.

1. This section does not apply to medication possessed by a student for self-administration under IC 20-33-8-13.
2. Medication that is possessed by a school for administration during school hours or at school functions for a student may be released to:
   1. the student's parent; or
   2. an individual who is: at least eighteen (18) years of age; and designated in writing by the student's parent to receive the medication.
3. A school corporation may send home medication that is possessed by a school for administration during school hours or at school functions with a student if the student's parent provides written permission for the student to receive the medication.

If you would like for your child to receive Over-The-Counter medications at school, they will need a consent form signed.

If your child does not have:

1. physician’s order for an over-the-counter medication or;
2. signed consent with authorization to administer by their legal guardian

your child(ren) will not receive medication. Verbal consent will not be accepted.
