Important Tax Information – Form 1095-C

SENSE Charter School will not automatically mail Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to employees.

Form 1095-C contains information about health coverage offered to you during the prior calendar year and may be used for your personal tax records.

You may request a copy of your Form 1095-C at no cost by contacting:

Upon request, SENSE Charter School will provide your Form 1095-C within 30 days of your request, or by January 31 of the applicable tax year (whichever is later).
